How do I create a new form?

To create a form, expand Registration in the navigation. Then click Create New Form.

Step One - Basic Form Info
Form Title, School Year, Registration Start Date, and Registration End Date are all required fields
  • Registration Start Date is the date that the form will become visible to the public
  • Registration End Date is the date that the form will no longer be visible to the public

Once all required fields are entered, click Continue to move to Step Two.

Step Two - Activities
Step Two allows you to choose which Genders, Grades, and Sports the form will be available.  Student Profiles that match this criteria will have the ability to submit forms based on this criteria. 
  • A Gender, and at least one Grade and Sport are required.

Once all required fields are entered, click Continue to move to Step Three.
Step Three - Form Fields
Step Three allows you to choose the data you wish to collect on the form.

There are 3 type of Form Fields
  1. Text - A text box will display on the form that allows a Parent/Guardian to type information
  2. Checkbox - A checkbox will display on the form that allows a Parent/Guardian to check/uncheck
  3. Date - A date picker will display on the form that allows a Parent/Guardian to select a date

Forms Fields in the "Selected Form Fields" section will appear on the form.  By default, we've populated the "Selected Form Fields" section with Student First & Last Name, and Parent/Guardian First & Last Name.

At least one Form Field is required. You can add, remove, reorder, edit, and create Form Fields.

To toggle between All Forms Fields, Student Info, Parent Info, and Miscellaneous Info, use the drop down menu.

To add a Form Field, click the right arrow for that Form Field.

 
To remove a Form Field, click the left arrow for that Form Field.

To reorder (selected) Form Fields, highlight a Form Field and use the Move buttons.

To create a Form Field, click New Entry.
  • Form Type, Field Type, and Field Name are all required
  • The "Required" checkbox is optional.  If checked, the Form Field is required for the Parent/Guardian to submit the form.
To edit a Form Field, highlight a Form Field and click Edit Entry.
  • Form Type, Field Type, and Field Name are only editable for created Form Fields.
Click Continue to move to Step Four.

Step Four – Signatures & Files

To add a signature or file, click the checkbox next to Add a Signature or File.
Required fields are designated by an asterisk(*).
  • Use the “Attachment” feature to add a file for your parents/guardians to download
  • Use the “File Upload Required” feature to require your parents/guardian to upload a file upon form submission
  • Use the “Parent Signature” and/or “Student Signature” feature to require a signature upon form submission
Additional Signatures & Files can be added by clicking “Add Additional Signature or File”.

Click Continue to move to Step Five.

Step Five – Fees & Donations
Step five allows you to add a fee (required payment) or a donation to the form.

To add a fee, select the checkbox next to Fee (Required Payment).

Once you completely finished with the form, click Create Form.
